Deployment Survey

L. Other Activity Survey Type

Use the Other Activity Survey Type to record any additional information that does not go into any of the other survey types. Some examples of instances when you would submit an Other Activity Survey Type are:

1. To record your departure location when you arrive at the airport

2. To record your accommodation on the way to, or back from, the disaster location

3. To record the team’s basecamp location
4. To record a place where you are sourcing fuel

5. To record your location in the event of a fuel tanker/vehicle breakdown

EXERCISE

The first survey that every volunteer should do when selected for a mission is the Other Activity Survey Type. This first survey is submitted on the day you depart (before you have left your home) to indicate that you are “preparing to deploy”. This is done so that FRF can record the total time spent and distance traveled on every mission we complete. NOTE – volunteers should use an approximate location, and not their exact home addresses for this survey.

For this exercise, complete a Other Activity survey (within the Training Deployment Survey) for “preparing to deploy” and click the checkmark at the end to submit it. Please make note of any questions that arise while you are completing the survey to review with your team’s EIM focal point prior to deploying.
