Deployment Survey

H. Distribution Survey Type

Use the Distribution Survey Type when beneficiaries are coming to you for fuel (i.e. when beneficiaries line up to receive fuel). Complete the survey once your team has finished distributing fuel to beneficiaries at a singular location. If the team distributes fuel at multiple locations in one day, a Distribution Survey Type must be filled out at each individual location once the team concludes distribution there.

1. After selecting the Distribution Survey Type, the first two questions are Distribution Start Time and Distribution End Time. This will help us understand how long you were distributing fuel at that location on that day.

2. If you distributed multiple types of fuel or to multiple beneficiary types at that one location, you will need to fill out a Repeat for each fuel/beneficiary type. To do this, click the Plus icon (see image to the right).

3. If you serve a beneficiary type that is not on the list in the survey, select Other and use the field that appears to state what the beneficiary type is.

4. You can add as many photos are you want for this distribution location. Once you have taken or uploaded a photo, a plus icon against the camera or folder appears (see image to the right). Click on either of these to add more photos.

EXERCISE

Complete a Distribution Survey Type (within the Training Deployment Survey) with the following information (for the location map, choose any location, it does not need to be your current location) and click the checkmark at the end to submit it:

The team gave out fuel from Summerfield police station between 8am to 4pm. The following beneficiaries received fuel:

1. World Central Kitchen (NGO) – 400 gallons of diesel for their generator. The organization serves 4,000 people and this is how many people their generator serves. The generator has a capacity of 400 gallons over 4 days.

2. Burnaby Fire Department – 40 gallons of diesel for 4 fire engines.
