Complete the Contact Survey Type to store deployment-related contact information in Survey123. The contact could be someone who FRF engaged with in the field (e.g. humanitarian aid workers, local government officials, etc.), or anyone else who might serve as a valuable resource for FRF in the future, such as a fuel industry representative that the team spoke to on the phone.
Contact Survey Type Repeat Section
Like all Survey123 surveys, Contact Surveys are location-based, meaning any contact you enter into the system will be associated with a particular location. The survey’s default location is where you are while you are entering the information, therefore, if you are entering contact information on the spot, you can leave the location where it is. If you are filling out the survey retroactively or are given the actual work address of the contact, you should manually set the location to where you encountered that contact or to their place of work (respectively). If you have contact information for more than one person at the same location, you will need to click the Plus icon (see image to the right) at the bottom of the Repeat to add another Repeat section.
1. Click No (see image below); and
2. Fill in the address in the field that appears. Be as specific as possible when entering their address.
